OPQRST-I is used to drill down into the patient’s presenting symptom—the reason they sought care. By asking about onset, what provokes or relieves it, the quality of the sensation, where it is and whether it radiates, its intensity, the timing and evolution, and any interventions already tried, you build a clear picture of the chief complaint. This focused questioning helps you understand the current problem in a way that guides triage and treatment decisions, rather than just listing past issues or unrelated history.
